First, you are planting under. I can tell this because your pole is bent prior to you leaving the ground. If you can correct this by moving back the start of your run, great, otherwise, you are in for a lot of plant box drills, etc.
The next thing is your trail leg is not straight as your go into your swing. Sand drills will help this. Lots of reps from 4 and 6 steps. Then go to the mat and see if it is better, and then repeat this process until it is perfect.
