Has Anyone Ever Had This Happen?

I know it would not be a popular "call" but from what I've seen in the pics and description, I'm afraid I would have told the meet director that I can not / will not officiate. Does not matter how important the meet is, it is not worth someone getting hurt. There must be someplace else that has a better / safer set up that everyone could go to.

exactly. my freshman year pole vault had to get postponed at the district/state qualifier meet. the meet had already been postponed once due to weather, then once we finally did get to have the meet, we had to have pv delayed again because the box was busted (...i think...it was something to do with the box).

Yep! It happened in my backyard setup. The problem comes from having a fiberglass box and no concrete or dirt behind it. On the brite side, the hole can be easily fixed with an automotive body filler called Tiger Hair or any resin based fiber filled body filler. Do a nice job of having the pole side of the box back board as smooth as possible. When this stuff hardens it's a bear to sand and make look nice. Lesson learned? Make sure you have plenty of support behind the box and don't just drop it in a ready made hole in a elevated runway or similar drop in application. The cost to fix mine? $28 fore the filler and about $10 for the ready mix bag of concrete. One more thing. Hole in the box? Don't jump. Later...........Mike
