I saw this episode today on the Discovery Channel. The girl is in college in Massachusetts and has a PR of 3.95m. Very high speed video - cool!!

Nice slo-mo vids! 
In this vid, you can easily see exactly how the pole hits the box, and how the pole swings to the left as it bends.
I'm tempted to critique her technique, but I'll restrain myself. She knows she was under, and she even said "you're supposed to takeoff before the pole hits the box".
She's hit the root cause (or maybe the ROOT cause is in the run), so everything after that is just salvaging a bad takeoff.
