It seems like most people get hurt because they don't take care of their bodies. I know that thousands of vaulters aren't warm-up well enough...they don't stretch AFTERwards, and they don't take ice baths at all. An ice bath is the best thing you can do after an intense workout...and hardly anybody ...

Exactly...get yourself on a highbar. Also do swing-ups on the rings. Once you do it on the highbar and rings, then you know you can keep a long trail leg. So then go back to vaulting and do it there.

Not too rare
The same thing happened to me jumping at Mount SAC a few years ago. The pole broke right underneath my bottom hand, and the top of the pole sling shot into my head. Blood was pouring down my head, so I took my shirt off and and applied pressure. I went to the ER and they put 9 staples in head.

No Tap.
The tap is a crutch. Why establish a comfort zone for something you can't use when the bar goes up? If you become dependent on the tap...then you're screwed.

Counting is a good idea, and if you're a seasoned vaulter I don't think it interferes with proper technique.
